Allocate less per request when presenting an entity or reading path params

Profiling the grape-on-rack app under live load showed four per-request
allocations whose only use was to feed another one:

- `present` collected the `env`/`version` embeds into a Hash and splatted
it into `represent`, whose keywords build a Hash anyway: two Hashes per
  call where one does.
- `present` built `[nil, object]` only to destructure it in the
  one-argument form.
- `Request#make_params` copied the routing args with `except` only to
`deep_merge!` the copy into the params. A capture is a String or an
Array of them, never a Hash, so the merge only ever assigned; the
captures are now assigned directly, skipping Grape's own keys.
- `format_from_extension` sliced the extension out with an endless
Range, an object of its own on every call.
