Restore support for downgrading of packages (issue #52)

This bug fix turned into a refactoring, which is why I bumped the minor
version instead of the patch level. The --download option implies
--ignore-installed but the changes in 0.26.3 (3292cbc088d) forcefully
reverted that logic. This made some changes from the pip 6 upgrade
irrelevant, hence the refactoring to clean that up.
