Fix aislop scan findings in the TUI

aislop reported three warnings over the whole tree. thread-view.ts was
478 lines, past its 400-line file limit, and carried two unrelated
concerns: building the thread chrome and routing the slash commands.
Move the command surface -- the command-name set and the dispatch switch
-- into its own module, so the view keeps the layout, lifecycle and
transcript it owns. The router takes a small context (host, sdk, threadId
and a callback for an unknown command) so the view keeps showHelp, and
ThreadView.runCommand stays a thin wrapper, leaving its one call site in
submit unchanged. The moved code is verbatim apart from hoisting the
no-argument command list to module scope.

The two empty catch blocks in the app's shutdown path now say why
swallowing is right: stdin may not be a TTY, and a closed stdout cannot
be restored anyway.

reconcileBlocks copied the container's children before removing them.
getChildren already hands back a fresh array, and remove mutates the
container's own list, so the copy was never needed.
