Raise test coverage from 86% to 98.6% of lines

Adds tests for the code paths that had none: the SDK wrappers (providers,
project lookup/creation, timeline paging and de-duplication, send, watches),
bbchat init across every branch including the default prompt() reader, config
file I/O under a temp XDG_CONFIG_HOME, shorthand resolution against a live
list, the default detached launcher, work-row titles and glyphs, and block
truncation and in-place updates.

The TUI view tests now cover interaction, not just first paint: composing and
sending, send failures, slash commands, Tab/Ctrl+E expansion, scroll keys,
coalesced realtime refreshes, and the terminals, spawn wizard, diff, skills,
and plugins flows with their empty and error states.

The release CLI is exercised end to end against a throwaway git repo per test:
plan's GITHUB_OUTPUT, docs-only and forced bumps, cut, notes, set-version, and
usage errors. main is exported for this; it still only runs under
import.meta.main.

runInit takes optional injectable detect/serverInfo/isTTY dependencies, the
same pattern detect and ensureServer use, so it can be tested without whatever
BB happens to be running on the machine. Defaults are unchanged.
