Refetch live views after a realtime reconnect

BB's realtime client reconnects on its own and re-arms its subscriptions,
but it never replays the changed events missed while the socket was down.
bbchat only watched thread:changed and project:changed, so a thread view
left open across a network blip showed a frozen transcript and a
permanently "working" status, with no key that could recover it.

Watch realtime:connection once at the app level and resync whichever view
is on top through a new optional View.onRealtimeResync hook. The hook
refetches without disturbing local state, so a resync landing mid-typing
keeps the composer draft and scroll position. Add /refresh in the thread
view as the manual escape hatch; a bare `r` key is not an option because
handleShortcut deliberately claims only non-printable keys, leaving every
printable character to the composer.

Also watch environment:changed in the diff view, so a commit made outside
the thread -- by the auto-review plugin, or by hand -- appears without a
manual refresh. The environment id is re-resolved on each refresh and the
watch is only re-opened when the id actually changes, so a thread that
moves between environments is followed rather than pinned to a stale one.
