feat(fabricx)!: rotate committer endpoints when a connection fails (#2249)

ClientProvider keeps one connection per configured committer endpoint
for the life of the node and hands out the first one, in configuration
order, that is not in TRANSIENT_FAILURE, or the first if all are.
Endpoints are dialled with AsyncConnect, so one that is down at startup
still gets a connection that gRPC keeps retrying, and startup does not
wait on it. For the same reason connectionTimeout has no effect on
these endpoints. ConnectionConfig.ClientConfig holds the mapping that
CreateGRPCClient and dial share.

The notification manager picks an endpoint on every reconnect. The
query service picks one per request, and retries once on a fresh pick
when a call fails with Unavailable: a connection whose server has gone
away reports IDLE until a call on it fails, so the first call after an
outage can still land on it.

The notification and query connection-state series follow the endpoint
in use. When another endpoint takes over, the previous one is reported
disconnected.

BREAKING CHANGE: NewRemoteQueryService takes a client factory that
returns the stub and its endpoint,
func() (committerpb.QueryServiceClient, string, error), instead of a
fixed client, and newConn takes the endpoint.

Closes #2247.
