Remove the obsolete Coveralls npm uploader (#1630)

- Remove the unmaintained coveralls development dependency and legacy npm run coverage upload command.
- Delete the obsolete .coveralls.yml token configuration.
- Remove the unused dependency chain containing request, har-validator, and uuid 3 from the local installation.
- Preserve c8 coverage generation, npm run test:coverage, and the existing coverallsapp/github-action@v2 CI integration.
- Document local coverage reporting and record the cleanup in the dependency maintenance audit.
