Scope meeting attendance to the congregation (#417)

Meetings had no account, so every congregation saw and could edit
every other congregation's attendance records. Meetings now belong to
an account, and both attendance controllers look them up through
current_account.

Existing meetings are assigned to the first account, using the same
AccountIdMigrationHelper as earlier scoping migrations.

Closes #391
