* tst_mainwindow: the Ctrl+C / OTP request state, end to end

Four tests drive the request state through the tree and the public slots:
an OTP request answers only its own entry and only once; the decrypt that
answers it keeps the password off the clipboard even in "always copy"
mode; the fast path copies the code already on screen without a decrypt
and leaves no request behind; cancelOtpRequest() forgets both the OTP and
the Ctrl+C request so a later decrypt of that entry answers neither.
