refactor(theme): lay the file out the way upstream does (#1252)

_theme.scss interleaved maps, functions, knobs and mixins, so the reader
met $theme-colors before anything that consumes it and theme-opacity-values
between two maps. Upstream puts every callable first and every map last,
and the file now does: theme-color-refs, theme-token-refs,
theme-opacity-values and theme-color-tokens, then the mixins, then
$theme-colors through $theme-state-colors.

generate-theme-classes() ends with generate-theme-reset() as it does
upstream, so a caller gets the classes and the escape hatch from one
include; helpers/_theme-colors.scss drops its second call. The upstream
headers on the three shared functions read as they do upstream, with the
token names in their examples carrying our prefix; our three narrative
comments go.

Compiled CSS unchanged: sorted coreui.css before and after has zero
differing lines.
