Build runtime hook engine and MCP action integration (#2076)

* feat: extend workflow hook types and validation for runtime execution

Add submit_for_approval and approve_task to WorkflowHookMcpMethod.
Add classification, order, and label fields to WorkflowHook.
Add WorkflowHookUserState for normalized hook result UI mapping.
Update hook validation to cover new fields.

* feat: build hook executor with sandboxed script execution

HookExecutor runs built-in and script validators.
Script execution uses Bun.spawn with restricted env, credential
stripping, timeout SIGKILL, and bounded stdout capture.
Built-in validators are registered by ID with stub defaults.

* feat: build workflow hook engine with chaining and result processing

WorkflowHookEngine snapshots matching hooks, sorts by classification
then order then id, and executes serially. Implements full result
precedence: block > retryable_block > patch_params > emit_follow_up
> record_state > allow. Includes wrapHandlerWithHooks for tool
handler integration with follow-up dispatch and recursive rejection.

* feat: wire hook engine into node-agent tool handlers and task agent manager

Add hookEngine to NodeAgentToolsConfig. Wrap send_message,
save_artifact, approve_task, submit_for_approval, and mark_complete
with hook pipeline. TaskAgentManager builds engine per sub-session
when workflow defines hooks.

* test: add unit tests for workflow hook engine

Covers allow, block, retryable block, param patch with re-validation,
follow-up action through handler pipeline, recursive follow-up
rejection, local state update, malformed script output, timeout,
multiple hooks on one action, conflict precedence, failure
classification, and normalized user-state mapping.

* fix: address P1/P3 review feedback on hook engine

- patchedKeys now diffs against original params instead of returning all keys
- Follow-up dispatch checks followUpRequest presence regardless of decision,
