[REF] Add functional test variants for all operators (#281)

Update functorch_gradient_and_loss to accept callable model_func.
Add to_functional helper that converts Module-based test cases to
callable + dict format. Add _functional test variants for all
operators (Hessian, GGN, EF, Jacobian, TransposedJacobian, GGN
diagonal) that run the full parametrized test suite with callable
model_func. Remove dedicated check_linop_callable_model_func tests.
