Allow specifying channel ID in extension view dialog (#158)

* Address variable usage issues

* Replace fetchManifest with fetchUserByName

The former is a wrapper around two API calls with additional custom
logic.  The latter is for a single API call.  The additional custom
logic of the former is now in the client method.

* Use camel case everywhere

* Improve manifest parameters

* Normalize "VideoOverlay" as "Overlay"

* Create and use a TokenSpec specification object

* Remove unnecessary use of channel ID

* Move channel ID into the extension view

* Allow specifying channel ID in extension view dialog

* Add a test for missing configuration errors
