Fix issue with local mode, remove extension-frame container (#161)

The frame proxy was expecting channel id to be set to the EXT_CHANNEL_ID. Update that path to take an explicit value for this rather than the coordinator params. Also fixed an issue with a path mismatch in local-mode.js after introducing twitch-api.
