refactor: address PR review on moderator flag check and request logger

- Moderator getModeratorAddresses now ignores the platform-user-moderators variant
  when the flag is disabled, even if it still carries a payload value, and a unit
  test covers the disabled case.
- Document why the request logger defaults a missing response status to 200.
