Close consent issue when fully resolved, reopen on drift

When every checkbox in the consent issue has been applied (struck
through), the issue now closes automatically — both eagerly from the
webhook flow when the user's final tick lands, and from the cron's
upsertConsentIssue when it observes the same.

If a closed consent issue later sees an unapplied or new id come back
in the plan (config change, drift reoccurring), it reopens with state
preserved. A closed issue whose plan is still fully applied is left
alone, so the cron does not flap state every 10 minutes.

upsertConsentIssue now searches for consent issues in any state, not
just open, to make the reopen path possible.
