Surface apply failures with ⚠️, untick, and a sanitised comment

When a consented change fails to apply, the bot now:

* unticks the box and prefixes the summary with ⚠️ so the user sees at
  a glance which item needs attention and has to retick to retry,

* posts a single comment on the consent issue summarising every failed
  item with its sanitised error message, and

* clears the ⚠️ (and adds the strike-through) when a later retry of
  the same item succeeds.

Errors are scrubbed against a conservative deny-list before posting:
GitHub PAT shapes, bearer tokens, Authorization headers, PEM private
keys, and named secret/token/key/password=value pairs are all
replaced with [REDACTED]. Messages over 4kB are truncated.
