Make markItemsApplied converge under concurrent writers

Ticking two boxes quickly fired two issues.edited webhooks that raced
on read-modify-write of the issue body — last PUT won, so an earlier
writer's strike-through was overwritten and only one item ended up
struck.

markItemsApplied now loops up to 5 times: re-fetch, merge our applied
ids with whatever any other writer has already persisted, and write.
The contribution from each writer is monotonically additive
(strike-throughs never get removed), so concurrent writers converge.
