* Make sequence diagram reports self-contained and interactive

The report loaded Bootstrap, jQuery, Raphael, highlight.js, clipboard.js
and js-sequence-diagrams from five CDNs, including an unmaintained page on
bramp.github.io. If any of them went away, every report broke, and reports
could not be viewed offline or as archived CI artifacts.

The report is now one HTML file that loads nothing from the network:

- The sequence diagram is an SVG drawn in Go, sized so every label fits
  between its lifelines. It renders without JavaScript, and each arrow
  links to its message.
- Requests and responses are listed with their start line, headers and
  body. JSON bodies are indented and highlighted on the server side.
- Requests that received no response, such as calls that matched no
  mock, are flagged in the diagram and the event list.
- A small inline script adds selection that links arrows and messages,
  filtering, focus on one participant, j/k keyboard navigation,
  collapsing, copy raw, copy as curl and a dark mode toggle.
- Light and dark themes follow the system setting, the layout works on
  narrow screens, and printing expands every message.

The public API is unchanged. SequenceDiagram writes the same file names
to the same directories, and the meta JSON is still embedded as
#metaJson.
