fix(playground): guard the disable branch of mcp toggle on response.success

The disable branch of handleToggleTool in mcp-server-selector was
unconditionally invalidating the cache and never surfacing failures.
Mirror the enable branch: only invalidate when the write persisted, and
toast on failure. Without this the UI would render the server as
disabled even when the DB still held the old enabled tools, and any
silent failure to materialise the thread row went unreported.
