Fix q.queue[0] bug; remove non-functional DECam telescope option

submit_too_ztf's per-queue-entry json dump always wrote q.queue[0]'s
content to every output file, regardless of which entry the file
was named after (a copy/paste leftover from before the loop existed).
Fixed to dump the loop variable. Strengthened the covering test to
use a two-entry queue and assert each file's content individually,
which the previous single-entry test couldn't have caught.

DECam was listed in model.py's all_telescopes, but was never
actually usable: set_default_filter's per-filter validation loop
only special-cases ZTF/WINTER and otherwise always raises, so
PlanConfig(telescope="DECam") failed for every input. Removed it
from all_telescopes, and the now-dead (telescope is never "DECam")
branch in plan.py that skipped --doAlternatingFilters for it.
