Harden the v2 router after review

- opencode-free refuses --service in any form (--service=x, or after a
  flag that takes a value) and gives a bare `session`/`auth` its own
  server.
- Failover: a request still on a model the turn left gets one immediate
  retry, then fails, instead of failing the turn or looping; the same
  bound applies to free/auto. A switch back to a model already left can
  fail over again, a half-done switch still spends the budget and leaves
  the error to surface, and a hung retry no longer blocks the session
  for good.
- A deleted parent or fork session no longer breaks routing for its
  children, and deleted sessions are forgotten.
- The free check counts cache prices, and a malformed price makes the
  cost unknown rather than free.
- router.log is kept at mode 0600, including logs from older installs
  and rotated copies; hook failures log a capped message and a few
  stack frames.
- e2e matches the assistant's reply event; tests cover the cases above
  and a model whose id differs from its modelID.
- Docs: install stop message keeps other plugins, 401/403 wording, the
  rollback steps for both free.jsonc formats, and what router.log holds.
