Split the plugin's server closure into named factories

server had grown to 185 lines, over aislop's 80-line limit. Move the
ranking and live-model cache, the model choice, the per-session turn
bookkeeping and the failover/resume pair into module-level factories
that take their dependencies explicitly, leaving server to wire the
hooks together. failover and resume stay in one closure because they
call each other.

No behavior change: every log message is byte-identical and all 30
tests pass unchanged. The two empty catch blocks now say why they are
empty, and the header comment no longer narrates what the code does.
