feat: rename Hand to Monitor and point it at Sentinel (#70)

Step 4 of the Hand-to-Monitor migration. The server side moved out of
Reach into Sentinel's Monitor module (sentinel#61, #62); this moves the
app to match.

- Projects, solution and namespaces: TheBleedingDeacons.Intergroup.Monitor*.
  Hand* types are Monitor*; Reach* types are Sentinel* (SentinelClient,
  SentinelResult, SentinelConfiguration), the config section is
  Sentinel:BaseUrl, and the MSBuild switches are Monitor*Only.
- The server contract: wp-json/sentinel/v1/, the sentinel_* error codes,
  and the monitor://auth callback, matching Sentinel's validator.
- A new app: org.thebleedingdeacons.intergroup.monitor on Android and iOS,
  the notification extension's bundle id now nested under it (iOS
  requires that; it was com.bleedingdeacons.hand.*), and a matching
  keychain group. Because nothing upgrades from Hand, the channel ids,
  preference and SecureStorage keys, sound and drawable are monitor_*.
- Firebase project sentinel-67404. CI reads the new
  MONITOR_GOOGLE_SERVICES_JSON / _PLIST secrets and MONITOR_BASE_URL, so
  main keeps building as Hand until this merges.
- README: what the app is for now, and who can sign in, which had gone
  out of date.

A handset running Hand uninstalls it and signs in to Monitor afresh.
