Refine coverage skill with root-cause guidance for gcov misses

Record that the multi-line variadic pack expansion and the publicly-constructible
Column guard are fixable (not artifacts), and that exception-landing-pad closing
braces and the loadExtension success path are the ones genuinely not worth chasing.
