Modernize the Coveralls badge URL to the github/.../master shields.io form

The legacy https://img.shields.io/coveralls/<user>/<repo>.svg path is
deprecated; use https://img.shields.io/coveralls/github/<user>/<repo>/master
so the badge explicitly tracks the master branch coverage.
