combine: '--keep-existing' to preserve local files

- Add new feature CLI option: 'ksconf combine --keep-existing'.  This allows
  files that existing in the TARGET directory to remain even though don't
  exist in any source directory.
- Add unit test for new CLI option.
- Update combine to show a more useful error message when a missing TARGET
  folder cannot be created.
- Expand docs and unit tests for match_bwlist().
