fix(sorted_set): add keeps an equal element already in the set

`SortedSet::add` overwrote the stored element when an equal one was added, while every other set (and `SortedSet`'s own constructors via `add`) keeps the existing element. It now leaves the set unchanged, which also avoids a write. Docs on `add`, `SortedSet(..)` and `from_iter` state the rule: of several equal values, the first is kept.
