* feat(chat): emit igcNavigate for links in messages and fix code colors

- The chat emits a cancelable igcNavigate with { message, href } when a
  plain click or Enter opens a link in a message in the same tab.
- The GitHub code colors use the --shiki-* variables with the GitHub
  colors as fallbacks, in light and dark mode. The chat themes no longer
  set these variables.
- emitEvent: an event with bubbles: false is not composed by default,
  so the igcNavigate of a tab no longer reaches outer shadow hosts.
- The tabs and the chat share linkOpensElsewhere() from internals.
  _parent and _top count as _self in a page that is not in a frame.

* fix(chat): handle SVG xlink:href links and nested chat headers in igcNavigate

- Match SVG links by xlink:href too, as the Markdown renderer does.
- A link in a nested chat, also in its header, belongs to that chat:
  the nearest chat in the event path must be this chat.
