feat: ToolCall.call_id

The id the provider or framework gave each tool call, filled from cassettes,
MockLLM, the framework adapters, and from_otel. Not compared, and shown in
failures. Tool calls without an id get one at replay, unique across runs, so
the framework and the result see the same id; mock_response keeps an id it is
given. Requested from Yamabiko (AV-3).
