ci: update actions, drop broken layer cache, restrict token permissions (#118)

The test and release workflows ran actions/checkout@v2, setup-python@v2,
coverallsapp/github-action@1.1.3 and satackey/action-docker-layer-caching,
all flagged by the runner as Node 20 actions forced onto Node 24, and still
emitted outputs through the deprecated ::set-output command. The cache step
has failed on every run with "Cache service responded with 400" since the
legacy cache API shut down, hidden by continue-on-error, and its action is
unmaintained. Neither workflow declared permissions, so every job ran with
the repository default of a read/write GITHUB_TOKEN.

Actions are now pinned by commit SHA to their current releases, the cache
step is gone, outputs go through GITHUB_OUTPUT, the token is read-only
except for the release job, and matrix values reach shell steps through
environment variables. Tests run on pushes to master and on the default
pull request events (opened, synchronize and reopened), with fail-fast off
so one broken optimizer no longer cancels the others, and the Coveralls
finish job runs even when an optimizer fails. Dependabot now also covers
the actions, the test Dockerfiles and each optimizer's setup.py. The
release action stays on v3 for now; migrating it is a separate change.
