Add a 404 response for a Laravel route parameter bound to a model (#13)

A route parameter bound to a model, such as `{post}` with `Post $post`
in the handler's signature, now adds a `404` response to its operation.
That is what Laravel answers when no row matches.

The parameter is matched by name or by its snake case form, as Laravel's
implicit binding does. One with a default gets no `404`, since the
default takes the place of a missing row. A `404` or `4XX` the
attributes declare wins.

Also, the adapters' class docblocks no longer list what a route
contributes, and point to the README instead.
