Describe a Laravel form request's input and its 422 (#14)

A handler that takes a form request now gets its input from the
request's `rules()`: a request body for `POST`, `PUT` and `PATCH`, query
parameters otherwise, and a `422` response.

Only rules with a plain schema equivalent are read (`required`,
`nullable`, the type rules, `min`/`max`/`between`, `in`, `email`, `url`,
`uuid`), with dotted keys such as `items.*.id` nesting.
`Routing\Laravel\ValidationRules` does the translation and can be
extended for a project's own rules.

`rules()` is called on a bare instance, outside a request. One that
needs the user or the route cannot be read, so the operation only gets
the `422`, and the adapter's new `logger` argument says why. A request
body or a `422` the attributes declare wins.

Builds on #13 for the response fold, so this targets that branch until
it is merged.
