Add optional XML escaping for literal values (#758)

This PR adds `escape=True` to `toxml()` for literal values and header
text. Forbidden XML 1.0 controls and U+FFFE/U+FFFF in escaped text raise
`ValueError`. Existing calls keep their output, including raw markup and
pre-escaped entities. Custom value templates remain unchanged.
