Tell the user when mcp:install configured a server that does not exist

Run in a real existing project, the command wrote a correct .mcp.json
pointing at `lemmego run mcp:serve`, reported success, and the editor
showed a red cross with no reason given — the worst available outcome,
because the thing is broken and nothing says why.

`lemmego new` registers the provider. An existing project has nobody to
do it: Go has no equivalent of Composer's package auto-discovery, which
is what lets Laravel's installer assume the server is there. So the two
halves — the module in go.mod and the provider in providers.go — are now
checked, and what is missing is printed in full, with the commands
rather than a link, because it is read at the moment the thing has just
failed.

The configuration is still written when the server is missing: it is
correct, and becomes useful the moment the provider is added.

Pins mcp v0.2.0; _scaffold/VERSION 2.16.0 -> 2.17.0.
