Test the mcp:install command and every validate rule

Coverage fell because the mcp:install command shipped with none: 0 of 34
statements in mcp.go, which is the whole file. It is 94% now, the two
remaining lines being os.Getwd and install.Run failing — neither
reachable without contorting the code for the number's sake.

The tests are behavioural rather than coverage filler. They assert that
installing writes an .mcp.json an editor can actually parse, with the
right command and args; that running outside a project refuses and
leaves the directory untouched rather than scattering configuration into
wherever somebody happened to be; that each opt-out flag omits its part
while .mcp.json is always written, since without it there is no server
to find; that --agent narrows correctly; that an unknown agent is
reported with the valid names, so nobody has to read the source to find
them; and that reinstalling is idempotent and preserves hand-written
content in CLAUDE.md.

validate also had two rules nothing exercised, including the one added
with the database session driver. Every rule has a case now, plus
working combinations that must not be refused — a rule that stops
refusing is a project that builds and does not boot, and a rule that
refuses too much is worse than no rule.

Verified by reverting: disabling the project check and the
database-session rule fails exactly the tests that cover them.
