feat(core): add trace ids to logs and stop duplicating the tool span (#423)

FastMCP already continues the caller's trace from the traceparent in the
request _meta, but logs had no trace or span id, so they could not be joined to
traces. _audit_wrap also opened its own SERVER span with the same name as
FastMCP's, so one call showed two "tools/call <tool>" spans.

- current_trace_ids() reads the active span through the OTel API (no SDK
  needed) and JSON logs and get_server_logs entries now carry trace_id and
  span_id while a span is active.
- The audit span is now an INTERNAL child named "execute_tool <tool>" under
  FastMCP's SERVER span, keeping all its attributes.
- Tests cover a valid, missing and malformed traceparent, including one that
  goes through a FastMCP client into a wrapped tool's audit log.

Kubernetes API spans are not added here because they need a new dependency
(opentelemetry-instrumentation-urllib3); that is still an open question on
the issue.
