Reuse a NATS consumer created between lookup and create (#6928)

If another process, such as a pod still running the old code, creates
the durable consumer after our lookup but before CreatePushConsumer,
the create fails with ErrConsumerExists because the configs differ.
Look the consumer up again and reuse it instead of failing Subscribe.

Also note that deployments using NATS subject permissions need to
allow _minder.> for the new deliver subject.
