test(utf8string): cover invalid UTF-8, leading BOM, and lone surrogates (#214)

Pin the current decode/encode behavior for edge cases the existing
fixtures don't exercise: invalid bytes fall back to the raw string with a
warning, a leading BOM is preserved, and lone surrogates throw on encode.
