* fix(signals): GRAPH_GROWTH keeps what navigations met for the session (#4101)

What outlived a navigation was the set of nodes reachable as that one
navigation opened. The walk reaches a long-lived source only through a page
that reads it, so a module-level signal (solid-refresh's per-component
registry, a global store) was in the set only when the page just left read
it too. Arriving at a route from pages that share more and more of it then
climbed in that count with nothing leaked. The same shape sat behind #4007
and #4035: the count tracked what the previous page shared with the route,
not what earlier visits left behind.

The nodes met as navigations open now go into one WeakSet kept for the
session. A long-lived source counts the same at every settle once met;
only what keeps accumulating climbs. Leaked roots, ownerless effects and
per-visit subscriptions still report, including variant F of #4007.
