auto-review: fix review findings in report tracking and the badge

- Only delete orphaned findings on a structured thread-not-found
  (code thread_not_found or status 404); a free-text "not found"
  match could wipe live threads' history during the sweep.
- Record outcomes best-effort so the latch reset always runs, record
  `stopped` when a reset hits a queued review, and make the
  not-reported hint match its cause.
- Pending reviews no longer show the previous outcome's time; the
  prompt-cap count covers open findings only and is exposed in status
  and the RPC.
- Count only applied findings, clear the dismissal reason on a fix,
  and ask reviews to leave carried-over findings out rather than
  re-report them; state the key and file input rules.
- Run the two periodic sweeps independently and log findings --clear.
- Badge: provenance-accurate aria-label, popover anchored to its badge
  and kept on screen, Escape scoped to the badge, non-modal region.
- Split findings.ts and prompt.ts into schema, store and prompt
  modules to stay under the file-size limit.
