auto-review: let the user end a held review and narrow what holds it

Review findings on the continuation latch:

- A user stop ends a held review, recorded as stopped.
- A turn of the user's own during or after the hold ends the review and
  is reviewed normally; the cursor is kept only when it was taken after
  the user's request, so a message queued or steered behind a review or
  wake-up turn never re-reviews the review's own fixes.
- Only wake-ups that are due now, not the user's, not a plugin's, and
  from no sender or a spawned descendant hold the review; an undated
  latch never holds.
- The provider gate counts a delivered review for up to the two-hour
  cap instead of the 30-minute stale window, so a sibling review cannot
  commit under fix turns still running.
- The continuation helpers move to review-continuation.ts, keeping
  evaluate.ts under the size limit; the docs describe the new behavior.
