dir-skills: write the work scope automatically on install

When ~/.bb/dir-skills/scopes.json is missing, a fresh install (bb.onInstall)
or `bb dir-skills setup` detects the work scope from data already configured:
the Claude profile from an ACP custom agent's CLAUDE_CONFIG_DIR, work remotes
from that profile's self-hosted git marketplaces, and the shared parent of the
matching bb project checkouts. It skips with a fixed reason whenever detection
is ambiguous, never overwrites an existing file, and keeps provider tokens out
of logs and output. The new scope applies without a plugin reload.
