fix(agent-status): keep OpenCode v1 blocked while another prompt still waits (#734)

the OpenCode v1 status plugin reported active on every permission or question reply. OpenCode runs tool calls in parallel, so two prompts can be open at once, and answering the first one cleared blocked while the second still waited.

the plugin now tracks pending request ids. A reply reports active only when no prompt is left. While one is still pending it reports blocked again, because the keystroke that answered the first prompt has already cleared the pane's status. A session's idle clears its pending prompts, since an interrupted turn drops them without a reply event.

tested by driving the plugin through Node with the recording wrapper, not against a live OpenCode v1.
