Move md_repr() into repr.h (#1778)

## What do these changes do?

Moves `md_repr()` and its walk visitor out of `hashtable.h` into a new
`_multilib/repr.h`, the same way `to_dict()` went into `to_dict.h` in
#1756. The code is moved unchanged; `_multidict.c` and `views.h` include
the new header. The C extension only; the pure-Python backend has no
equivalent.

`tools/codegen_diff.py` against the merge base reports no function
emitted differently on either build.

## Are there changes in behavior for the user?

No.

## Is it a substantial burden for the maintainers to support this?

No; it shrinks `hashtable.h` by about 160 lines.
