Fix distinct() with a count on header-only tables (#741)

This PR has the objective of fixing `distinct()` when you ask for a
count on a table with a header but no data rows (an empty CSV export,
say, or whats left after a `select()` that matched nothing). Without
`count` you just get the header back. With it,
`list(etl.distinct([('foo', 'bar')], count='n'))` on 1.7.28 dies with
`TypeError: 'object' object is not iterable`, because the bit that
writes out the last group runs even when there was never a first one.
