fix: honor explicit -o over config output folder and timestamp (#31)

An explicit -o path was rewritten whenever context-builder.toml set
output_folder and/or timestamped_output, so the file landed in a
cwd-relative docs/ directory instead of the path the user passed.
Config output_folder and timestamped_output now apply only when -o is
omitted, and a relative output_folder is resolved against the project
root (-d) rather than the process working directory. -o - still writes
stdout.
