fix: grow backtick fences past runs inside file content (#30)

A fixed ``` wrapper closed early whenever a file contained its own
fence. Choose a fence at least one backtick longer than the longest
run in the body, minimum 3, for file contents, auto-diff bodies, and
unified diffs.

Paths rendered as inline code use a delimiter longer than any backtick
run in the path, with CommonMark space padding when the path starts or
ends with a backtick or a space.
