e-stepper: every name lookup is a step of its own (#524)

A name was looked up implicitly, as part of the step that used its value,
except when a later operand could change its binding (e.g. in a while
loop's predicate i < f(x)), where it got a "Looking up i" step. Now every
name in the program is looked up in a step of its own, left to right:
"Looked up balance in frame E1: 100". Names of builtins and library
functions, which no drawn frame binds, are still looked up as part of the
step that uses them.
