* fix(agent): index in-flow icon controls that have no accessible name

A pointer div whose only content is an svg never entered the element table, so the agent could not click a gear or a star.

A disabled button is not indexed, so the icon inside it was offered on its own. The ancestor walk still keeps an icon whose unindexed parent is enabled.

An aria-label on the svg is a name, and a size class such as w-4 is not.

A leading class named icon was winning over ruyi-icon-bell, so different icons shared one label.
